NOTICE OF PRIVACY PRACTICES FOR iClinic Healthcare (PRIVACY POLICY)

We, at iClinic Healthcare, respect the privacy of each and every person and are committed to protecting your personal and medical information. To better protect your privacy, we provide this notice explaining our policy within this area.

I. What information may be collected?

In order to use iClinic Healthcare, you are asked to create a user ID, enter an e-mail address and password, which is referred as your iClinic Helathcare ID or credentials. After ID is created, you can use the same credentials to sign in to this service. You can manage your profile and/or account; search physicians; submit documents (including previous scans and documents) and ask specific queries addressed to the chosen physicians.

As any hospital, iClinic Healthcare also creates and maintains a record of the care and the services you receive. The record includes your history details, uploaded data and image files, consultation record, video files associated with all consultations, electronic medical records that may be uploaded or created as a result of treatment, medical test results and prescription records.

II. How is your information used by iClinic Healthcare?

We may use and share your medical information as listed below. Not every possible use or disclosure will be listed and all endeavors have been made to list down possible uses:

  1. We may share your medical information with our service providers like doctors, nurses, technicians, students, operations and maintenance team repairing and maintaining our technical systems. Your medical information may also be shared with our partner firms that may provide you with medicines, laboratory assistance, transcription services, etc. Please note that iClinic Healthcare does not have Non-Disclosure Agreements with these concerned parties who may be able to access your information.
  2. In process of providing you services we may be required to use and share your medical information with others who help pay for your care, such as insurance firms.
  3. iClinic Healthcare may use your information to study results and produce reports. Such information may be shared with iClinic Healthcare partners and advertisers under the sole discretion of iClinic Healthcare.
  4. We will share your medical information when required to do so by law enforcement agencies and under any state or local law. In the case of danger to self or others and in the case of any legal investigation, iClinic Healthcare will have the right to share your information with the law enforcement authorities.
  5. We may share your information for operations that help us run our programs and ensure quality care to our patients. For instance, we may use your information to review our services, performance of our staff and for teaching purposes.
  6. We may use and share medical information to inform you about different types of treatment available to you. We may use and share medical information to inform you about other benefits and services related to your health.
  7. Unless you specify otherwise, we may share your medical information with a family member or friend who helps with your medical care. We may share your medical information with a group helping with disaster relief efforts.
  8. With a view to promote research, development and enhancement of e-Health initiatives, we may use and share your medical information for research. Your information shall be so used only with your prior consent and the used data would not expressly identify you in any way.
  9. We may also share your medical information under special situations:
    1. We may share your information with governmental agencies that oversee healthcare and for public health activities.
    2. For activities approved by law such as audits, investigations, license surveys and with government registries.
    3. To notify concerned legal authorities if we believe a patient has been the victim of abuse, neglect or domestic violence. This information is shared only when ordered or required by law.
    4. Your medical information may also be shared with law enforcement agencies officers in case of national security.

III. This website contains links to other sites. Please observe that we are not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every website collecting personally identifiable information.

IV. We will be sending emails at the email ids provided by you and we will be making calls on the phone numbers provided by you if required. In case you have given a wrong email id or wrong phone number for correspondence, in that case iClinic Healthcare will not be responsible for the information loss or privacy loss due to the message going to an another destination.

V. You may ask us to give you a copy of this Notice at any time. Even if you have agreed to get this Notice electronically, you still have a right to a paper copy of this Notice.

VI. Revisions to This Notice: iClinic Healthcare reserves the right to revise, modify or amend this notice/ privacy policy at any time. The revised notice/policy shall be updated on this page with immediate effect.
